APPLY FOR A RE-EXPORT PERMIT

1. Fill out the CITES Re-Export Application Form 3-200-73 and follow all instructions provided in this form.

2. Please note that additional documentation will be required such as:

a. A Copy of the cancelled CITES export or re-export document issued by the appropriate CITES office in the country from which the wildlife was imported

b. A copy of the cleared Wildlife Declaration (Form 3-177)

c. A copy of the importer's documents outlined above and the Invoice or other document that shows you acquired the wildlife from the original importer or history of transactions that demonstrate chain of ownership
